Kindergarten Screening News-The children that will be attending Kindergarten in August will have their screening appointment attached, along with a physical form and a dental form.  All children entering Kindergarten need a current physical on the correct form making sure it is complete.  Prefer by the start of school.  Dental screening needs to be completed for all Kindergarten students.  All children need to have updated immunizations. These are the 4-6 year old shots.  DtaP, IPV(polio),  MMR, and Chickenpox.  The state has also required an eye exam by an optometrist, (this is new this year)  We have not received any forms for this yet.  The optometrist should have a form for you.  Please make sure you get a signed copy.  Pre-K will send your childs birth certificates, social security card, and lead screening.

Returning Pre-K Student News-If your child is already participating in the Pre-K program this year, you will have to register your child at the grade school registration in August, for the 2008, 2009 school year.  Your child will not have to be re-screened by Four Rivers, unless you are notified to do so.
