My name is Julie Bland and I am the certified school nurse for the Carrollton School District. My employment with the school district started in the 2003-2004 school year. I graduated from St. John’s College, School of Nursing, in 1997 in which I received a Bachelor of Science. I continued my education at SIUE and received a Type 73 School Nurse certificate in 2001.
HEALTH RECORD REQUIREMENTS
All Kindergarten need:
Current physical
Current immunizations
Lead screening
Dental examination
All 5th Graders need:
Current physical
All 9th Graders need:
Current physical
Tetanus booster immunization
All Kindergarten, 2nd and 6th Grade students need:
Dental examination
All transfer students from out of state need:
Current Illinois physical
Record of immunizations
Physicals and immunization records must be turned in to the school prior to October 15, 2007. If a student does not comply with this requirement that student will not be allowed to attend school until they fulfill this requirement. Dental examinations must be completed prior to May 2008.
